Service Coordination

Targeted case managers act as service coordinators.  They can assist in making sure that your basic, educational, vocational, and social needs are being met, so that you can focus on your mental health goals.  We find that many people are not aware of all the resources available in their communities and need help locating and navigating the various systems.  Some things that a service coordinator can assist you with include:

  •      *Linking to employment opportunities.

  •      *Referring you to mental health or substance use services such as individual, group, and/or family

  •      *Identifying and assessing you or your family’s needs.

  •      *Helping you identify supportive people in your life.

  •      *Linking you to resources for food, utilities, and other basic necessities

  •       may be supporting you.
