Service Coordination
Targeted case managers act as service coordinators. They can assist in making sure that your basic, educational, vocational, and social needs are being met, so that you can focus on your mental health goals. We find that many people are not aware of all the resources available in their communities and need help locating and navigating the various systems.
Some things that a service coordinator can assist you with include:
- Assisting with locating and securing housing.
- Linking to employment opportunities.
- Referring you to mental health or substance use services such as individual, group, and/or family therapy, AA, NA, parenting classes, etc.
- Identifying and assessing you or your family’s needs.
- Helping you identify supportive people in your life.
- Linking you to resources for food, utilities, and other basic necessities
- Helping to navigate social support services such as: Social Security, Disability, GED classes, College Applications, Food Stamps, etc.
- Collaborating with members on your care team to ensure you receive the best care possible. This team may include your therapist, community supports, primary care physician, psychiatrist, nurse practitioner, social services, department of juvenile justice, a teacher, or anyone else that may be supporting you.
- Collaborating with school officials to help address behaviors, peer concerns, IEP related matters/implementation.
- Linking to social activities (YMCA, Boys and Girls Club, exercise programs, camps, sports, 4-H, etc.)
- Identifying and linking with other support services such as the Michele P. Waiver, SCL waiver, and others.
- Monitor quality and access to therapeutic and other needed services.
- Meets with you several times monthly to ensure that needs are being met, resources are being utilized, services are going well, and continually assessing your needs.
- Must have KY Medicaid to qualify for Targeted Case Management
